Office Visits

Please see the list of our common fees for the practice. We are currently (out-of-network) not accepting commercial insurance and are not a Medicare or Medicaid provider. We are able to issue a "Superbill" which can be submitted to your insurance company for possible reimbursement. This is up to you. Please check with your insurance company. Our decision to not accept insurance is a difficult one and is based primarily on the cost of this to both us and our patients. At this time, we feel as though we can offer competitive (lower) rates by bypassing insurance. We do realize that this may be difficult for some clients and we are willing to offer a sliding scale on a limited basis. Please discuss your particular circumstances with our staff.

 

Initial Psychiatric Evaluation: ***$250

These usually take between 45 to 90 minutes and involves gathering a complete psychiatric and health history including developmental history, family and social history, substance abuse history, past medications and treatment trials, assessing for current symptoms. At this visit, we will work on developing a treatment plan together. This evaluation must be done in person. Medications are not guaranteed at this visit.

Follow-up Visit/Med Management: $150 to $250 depending on time and complexity (typically $150) is a 30–60 minute visit

This is where your psychiatric medications will be reviewed and evaluated for effectiveness as well as possible side effects. Your overall well-being and safety will be evaluated at these appointments and recommendations/referrals will be given as needed. These appointments may be done virtually on a case-by-case basis. Support/therapy are included in these visits.

 

Simple Med Check: $75

This is a 15 minute visit for establish patients who only need medication management/medication refills. 

Report fee: $60

Other activities such as letter writing, telephone calls, filling out forms, etc. $25 per 15 minutes.

The State of Minnesota requires an annual visit for new medications as well as medication re-fills.

Payment Policy

Services are expected to be paid in full at the time of appointment. Appointment may be cancelled or rescheduled 24 hours prior to the appointment without penalty. Charges for appointments cancelled less than 24 hours will be subject to a $35 fee. We do ask for you to provide your credit card information at the time of scheduling for a pre-authorized payment but will not charge it until your scheduled appointment. For your convenience, we accept major credit cards, cash, check and HSA/FSA. Primrose Mental Health PLLC does not accept insurance, if you have out-of-network benefits, you may request reimbursement from your insurance provider. Please Request a detailed statement or “Superbill” if you wish to file a claim. If you are struggling to pay your bill, please discuss this with the clinic staff as a payment arrangement might be possible. If your account becomes delinquent, we may refer you to a collection agency.

*** Starting at $250 for 45 minutes, $300 for 60 minutes and additional $50 for every additional 15 minutes.
